  3. Transcript notes. A list of what most transcripts include, plus a course description page for each elective.
  4. A faith-based business elective. A 12-week outline for a half-year course your teen would run under your oversight.
  5. Arizona ESA documentation worksheet. A worked example and a page for your own course of study.
